Hreflang without the headaches
Every version must link to every other version
Hreflang only works when the annotations are reciprocal. If your German page points to the English one but not the other way round, search engines may ignore both.
Include a self-reference
Each page's set of tags should list itself as well as its alternates. The simplest approach is to paste the exact same block into every version.
Language first, region optional
Use just a language code when one page serves all speakers of that language. Add a region only when you really have different content, like prices in GBP versus USD.
Pick one method and stick to it
HTML tags, sitemap entries and HTTP headers all work, but mixing them makes errors hard to track down. Sitemaps tend to be easiest to maintain on large sites.
